We are pleased to share with you an update of our activities to date which includes an updated summarized version of the strategy that has been endorsed by the South East LHIN and reflects feedback from the webinar and survey completed in June 2016.
In 2015, an environmental scan of exercise, falls prevention and management programs and services was conducted in South East Ontario. A survey tool was developed to collect data on key elements of each initiative and was administered by email and phone to contacts in the community and healthcare settings serving older adults.
The 2017 to 2018 Annual Report highlights the current commitments, values and assets of the Centre. This includes information about our team, activities, partnerships, funding requests, recent presentations and workshops, and future strategic opportunities.
Primary Care Clinical Resource Toolkits were designed as part of the Geriatrics, Interprofessional, Interorganizational Collaboration (GiiC) project. Its purpose was to support family health teams and community health centers in the delivery of care for frail seniors.
The resources offer a systematic and inter-professional approach to common care issues through the "identify, prepare, evaluate, action" framework. This basic clinical approach can be used for the evaluation and management of any chronic condition.
